We are a team of Canadian respirologists and health care providers with a goal to determine whether early diagnosis of undiagnosed chronic obstructive pulmonary disease (COPD) or asthma and subsequent treatment will improve participants' quality of life.

Our group has developed a questionnaire for Canadian adults to complete in their home to see if they are at risk of having asthma or COPD and are willing to participate in a research study.

This questionnaire is for you if you are an

• Adult - 18 years of age or older and
• Currently experiencing or have experienced respiratory symptoms such as shortness of breath, wheezing, increased sputum, or prolonged cough in the past 6 months

This questionnaire is not for you if you already have a diagnosis of asthma, COPD, or any other lung disease or you are currently taking a respiratory inhaler.
